I am presently in a relationship of four years with plans of a wedding in the near future.

I have resided in Pittsburgh, Pennsylvania area for the last 29 years.

I have a Spaniel/Papillon mix service dog named Jasmine. She is the friendliest, loving dog and mascot of my high rise.

I enjoy doing many different crafts.

I make lap blankets for the Three Rivers Hospices program. I have been doing this project for five years.

I started Crafty Corners in Crafton Plaza Apartments. I teach and help senior citizens and disabled tenants various crafts.

I have the responsibility of decorating the vestibule area of the floor which I reside. Changing it monthly or more often depending on seasons.

Disability Issues

Before the correct diagnosis I was treated for Hodgkin's Disease, Parkinson's Disease, Muscular Dystrophy, Multiple Sclerosis. After several years found that my disability was caused by Hereditary Spastic Paraplegia with Peripheral Neuropathy.

This disease is similar to Multiple Sclerosis but works from the inside of the muscles and nerves. By the time it is detected the tissue is already dead.

As a result I have a very hard time standing, showering, or walking short distances. Which has me confined to a power wheelchair.

I have torn biceps caused by using my arms for transferring from and to my power wheelchair.

I have other medical issues including Depression, Arthritis, Epilepsy, Severe Leg Spasms, Weight gain due to inactivity which was corrected through Gastro Sleeve Surgery.

I have trouble transferring from my power wheelchair to vehicles without help. As my vehicle is not adaptable for accessible wheelchair driving. So I rely on others to take me to my many doctor appointments and shopping trips.

Financial Hardship

I am currently in desperately need of a power wheelchair accessible and driving adapted van. As I can no longer transfer into or out of my vehicle without help due to the progression of my medical conditions.

At present time I have to wait for someone to be available to take me to appointments and errands.

This is why I need an accessible wheelchair van which is adapted for my driving needs as per doctors and the University of Pittsburgh, Department of Rehabilitation Science and Technology, Center for Assistive Technology.

Income Efforts

I presently receive Social Security Disability. The amount received is barely enough to cover expenses not including the monthly payment of a new vehicle loan.

I presently receive Food Stamps of $98, Housing and Urban Development housing subsidy, and monthly Food Bank donations.

Specific Needs

I need funding to get an adapted van to fit my needs as set forth by doctors and Center for Assistive Technology.

I also need grants or other assistance to help with debt caused by using credit cards to help make ends meet from time to time.

Hello Timothy,

Congrats on getting your story published! In doing so, you've acquired valuable hands-on experience in responding to precise instructions, the very kind that you will encounter when applying for help of any kind.

You've now earned the right to access the Workbook, which shows you how to use your story as a springboard to getting the help you need.

Cheers!
