Grant For Disabled Mother To Improve Home Accessibility

I am a 47-year-old female.
I have one son that is retired at the age of 26 after serving in military. He served in Iraq from 2006 to 2007. He suffers from post-traumatic stress disorder.
My family fell apart. After my parents went to an assisted living facility, my sisters abandoned me. Life became very stressful and complicated.
I try to contact my sisters, but they don't call me much.
I have many disability issues.
I have brain problems. I recently found this out after 47 years.
I have unspecified pervasive developmental disorder. This means I am partially autistic.
This characteristic is as much a part of me as being part Finnish, French, and Norwegian. I was born this way and I cannot change myself. I have to adapt. I need to accept myself for who I am.
I have physical disabilities. I wear braces on my legs and wrists. I use a cane crutch or walker.
I have other mental disabilities. I'm learning disabled. I also have schizophrenia.
My problems started in childhood.
I'm living in an assisted living apartment. My home is not totally accessible for the disabled.
I cannot afford to move. This is best place I can afford. Utilities are included in my rent.
I get SSDI, but my rent takes half my income. I do work but I only make about $65-$100 each month.
I have looked for grants on the internet. I also contacted the Minnesota state governor’s office to see if they offer any government grants.
I am working as much as I can. Unfortunately, I get sick a lot.
I have asked family, friends, churches, and the county for help. No one has money to donate to my cause.
I am looking to get a grant to make my apartment handicap accessible.
I need new updated windows because the window glass is single pane. The glass is cracked and taped with packaging tape. The window frame is rotting and the paint is peeling.
The exit is a dangerous set of stairs. I have almost fallen a few times. I really need a lift in my entryway, but they are very expensive.
