Grant Money to Help Make Ends Meet For Single Mom Of Disabled Twin Sons

I am a single mother of three great kids. My daughter is 13 and my twin boys are 11. I am 38 years old.
I have never been in the military. I do not have many family members nearby. I was in foster homes growing up.
My twin boys are the ones who have disabilities. They are both bipolar and they both have ADHD.
They have both been on four different kinds of daily pills since they were five years old. Their pills are changed every year when they start school. They will be going into 7th grade next year.
The disabilities come from their father's side of the family. They do not have any contact with their father, although he can see them anytime.
Since I am a single mother of three kids, it is very hard to pay for what they need. I fall behind on all my bills. I have a car I am still paying for.
I just went back to work after taking time off for one of my sons to get his medication right. Being off work has really hurt.
I have to have a job that does not work me 12 hours a day or schedule me third shift. That way I am there when my kids get home from school.
I have a job, but have not started making money yet. One of my boys is getting SSI and I have filed to have my other son on there. I have not heard anything back yet.
I have one brother here in town that helps me out when he can. He has a family to take care of too. I have asked friends to help, but no one is willing to help. I go to church and they have helped me when they can.
I am in need of benefits, grants, and SSI for my other son. I am in need of a lot of things and I really don't know where else to turn.
I will take any advice that anyone is willing to give. I am open to anything and everything. Please help.
