53 Year Old Permanently Disabled Divorcee Looking For Government Grant Money

I'm a 53-year-old permanently disabled divorcee looking for government grant money to help with everything, but especially transportation.
I suffer from Fibromyalgia, Spinal Stenosis, IBS, Degenerative Disc Disease, Muscle Spasms, Migraine Headaches, every other day, sometimes several in one day.
I live in my own home that I bought back in 1996 (I wasn't disabled then) and since have done quite a bit of work on it. But being disabled now since 2003, I find myself in need of a bathroom that will allow me to enter with a electric chair. The bathroom I have now is not accessible with a wheelchair.
It is a narrow door and the walls are concrete with my tub right against the door jam. This makes it impossible to move unless I tear out the tub, which I cannot do because I need it.
My car has just blown a head gasket. Sadly, it's my only mode of transportation for monthly doctor appointments, which I cannot miss. If I do I won't get my prescriptions for the overall pain that goes along with Fibromyalgia, which is very painful.
I have to go to the grocery stores for my food and my medicines. I need to have my car fixed but I don't have the funds to do so.
I lost my Mom last year and my brother is not financially able to help me as he has a family of his own with a son serving in Iraq. This is my last hope of getting the help that I need.
I am not one to ask for hand outs. I have worked since I was 16, going to school half a day and working the other half, and my father was a abusive alcoholic.
I'm pleading with those able to help me acquire the funds needed to take care of the things I need and cannot afford. A government grant for transportation or any equivalent help would be greatly appreciated.
